SUMMARY:   The MH LPN is responsible for working with the Director of Health Services in the delivery of nursing care and overseeing all health concerns within the agency by performing the following duties.

EDUCATION and/or EXPERIENCE:  Graduate of Accredited Practical Nursing Program and hold an active Pennsylvania LPN license.

Vital Duties:  Duties the employee performs that have a major impact on the delivery of services to our clientele

  • Ensuring appropriate storage and monitoring of medications.
  • Assisting with implementation of physical therapy guidelines.
  • Assisting in the scheduling, transportation and completion of dental, vision, audiology, and laboratory exams and evaluations.
  • Assisting in the scheduling of appointments with Occupational Health as necessary following assessment of work related illness, injury and significant exposure incident.
  • Documentation and monitoring of client health statistics (height, weight and waist circumference) on a monthly basis.
  • Providing training to the individuals and staff regarding health and hygiene methods, infection control procedures, detection of signs and symptoms of illness.
  • Assisting in the ongoing education of all non-licensed staff that administers medication.
  • Completing admission and discharge documentation, as well as ongoing health related assessments and documentation.
  • Administering and monitoring Hepatitis B Vaccine series and Mantoux testing of new employees.
  • Responding to "on-call" or as needed physical health requests and restraints both at the office location and in direct care facilities after regular office hours.
  • Addressing daily health care needs of the individuals and taking needed action to address the individuals' health problems including physician referrals.
  • Administering client medications.
  • Maintaining and stocking of medications and first aid supplies including PPE's.
  • Overseeing medication management of clients; including but not limited to, transcribing orders, communicating clients' needs to physicians, receiving orders, ordering of medication, monthly med cart inspections, and weekly MAR reviews.

PROBATION EXPECTATIONS: Must demonstrate during probation competencies in the following areas:

  1. Working with MH/MR Clients
  2. Completing Nursing Assessments
  3. Completing Medication Related Review Activities
  4. Scheduling Medical Appointments
  5. Ordering/Stocking Medications
  6. Overseeing Medication Management of Clients
