SUMMARY: The Community and School Based Behavioral Health (CSBBH) Professional is responsible for coordination of treatment for their assigned caseload by performing the following duties.

EDUCATION and/or EXPERIENCE: Master trained, Licensed in Social work, Counseling, Psychology or Nursing. Management experience required, plus two years experience in a CASSP systems program or be a licensed registered nurse with five years of experience, including two in CASSP system program plus certification by the American Nurses credentialing center (ANCC) or American academy of Nurse Practitioner (AANP). Certification as a National Board Certified counselor is also acceptable.

Vital Duties:  Duties the employee performs that have a major impact on the delivery of services to our clientele

  • Documentation of important information and data in a timely and professional manner as prescribed by the program director.
  • Preparing Treatment Plans, Discharge Plans and reviews, and making recommendations with regard to treatment planning with clients and their families.
  • Ensuring that agency stipulated productivity levels are obtained on a weekly basis.
  • Completing MA/Managed Care pre-certification and continued care requests in a thorough, accurate and timely manner.
  • Coordinating all aspects of treatment for the clients and their families within their caseload in a professional and timely manner.
  • Completing Comprehensive Assessment to determine need of SBBH within 48 hours of referral.
  • Ensuring services are initiated within 1 day of verbal authorization by MCO.
  • Ensuring initial SBBH packets are submitted to MCO within 5 business days after ISPT.
  • Completing Assessments, Crisis Plans, and Reassessments as needed.
  • Using individual and family therapy when appropriate as a means of treatment. Preferred knowledge includes family systems theory and application, social ecological theory and application, behavioral therapies theory and application, cognitive-behavioral therapy theory and application, pragmatic family therapies theory and application, , and social skills assessment and intervention.
  • Running Supervision meetings weekly for a minimum of 2 hours with each team to include the following topics:
  • Discussion of roles in the family
    • Triangulation in family relationships
    • Building alliances
    • Supporting parents
    • Genogram depicting family history
    • Ecomaps determining strengths of resources
    • Areas for advocacy
    • Mental health diagnosis of identified client and impact on family systems
    • Community strengths mapping
    • Transference and counter transference
    • Role of team in family system
  • Ensuring completion on a monthly basis and at the end of treatment   Community Care's Child Outcome Survey (COS).
  • Ensuring completion of the Strengths and Difficulties Questionnaire (SDQ) by families and teachers, at admission, quarterly and at completion of services.
  • Participate as a member of the SAP Core Team.
  • Direct supervision and management of the daily functions of the department. Which includes
    • Interviewing and hiring new staff.
    • Ensuring that new employee and annual evaluations are thorough and given in a timely manner.
    • Effecting any necessary employee comments and dismissal action in conjunction with the Director of Resources.
    • The training of new employees and on-going on-the-job-training with all staff.
    • Reviewing all time sheets to ensure they are properly filled out and submit them in a timely manner for staff assigned to his/her department.
    • Scheduling staff working hours and assigning staff to areas to provide adequate coverage within the Agency's requirements without previously authorized overtime.
    • Periodic direct observation of staff performance and providing feedback to staff on their performance.
